Magnit Global zoekt een Wmo-consulent voor de gemeente Oss. Je voert gesprekken met inwoners die ondersteuning nodig hebben, onderzoekt wat iemand zelf kan en welke voorzieningen beschikbaar zijn. Je stelt indicaties op en legt doelen vast voor zorgaanbieders.
Je werkt in een betrokken wijkteam met zorgpartners en zoekt gezamenlijk naar passende oplossingen. Je krijgt ruimte om je werk zelfstandig vorm te geven en hebt direct invloed op het dagelijks leven van inwoners.
