Bluestones Medical Nursing Agency is currently recruiting four experienced Community Nurses to work within Shropshire Community Health
, with:
- 2 vacancies based in Shrewsbury
- 2 vacancies based in Telford
This is an excellent opportunity for experienced Registered Nurses looking to work within community healthcare services, providing high-quality nursing care to patients in their own homes and community settings.
The successful candidates will work as part of a multidisciplinary team to support patients with a range of clinical needs, helping to promote independence, prevent hospital admissions and support safe discharge and ongoing care within the community.
Key Responsibilities- Provide high-quality, compassionate and patient-centred nursing care within community settings.
- Assess, plan, implement and evaluate individualised patient care.
- Visit patients in their own homes and other community settings as required.
- Monitor patients' conditions and identify any changes or deterioration.
- Administer medications and treatments safely in accordance with clinical guidelines.
- Undertake wound care, dressings and other nursing procedures within your scope of practice.
- Support patients with long-term conditions and complex healthcare needs.
- Recognise and upscale concerns regarding a patient's health and wellbeing.
- Work collaboratively with GPs, community teams, hospitals, social care and other healthcare professionals.
- Support hospital discharge and prevent avoidable hospital admissions where appropriate.
- Promote patient independence, self-management and wellbeing.
- Provide education and advice to patients, families and carers.
- Maintain accurate, timely and comprehensive clinical records.
- Ensure effective communication and handover between members of the multidisciplinary team.
- Support safeguarding procedures and elevate concerns appropriately.
- Follow infection prevention and control procedures.
- Comply with relevant Trust policies, procedures and clinical guidelines.
- Ensure all care is delivered in accordance with the NMC Code and professional standards.
- Current NMC registration as a Registered Nurse.
- Recent and relevant experience working within a community, district nursing or equivalent nursing setting.
- Experience providing nursing care to patients in their own homes or community environments.
- Strong clinical assessment and care planning skills.
- Ability to work independently and manage a caseload effectively.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a multidisciplinary team.
- Commitment to delivering safe, compassionate and patient-centred care.
- Current mandatory training relevant to the role.
- Ability to travel between community locations as required.
- Right to work in the UK.
- Satisfactory compliance and employment checks.
