In this position, you will work collaboratively with a multi-disciplinary team to provide primary healthcare to individuals and families within the PQCHC area. Your actions will directly contribute to addressing health disparities among underserved populations by delivering personalized care while managing both acute and chronic medical conditions. You will also facilitate health assessments and develop actionable care plans.
Key Responsibilities:
• Offer primary medical care aligned with CPSO standards
• Manage acute and chronic conditions across various age groups
• Partner with health professionals for comprehensive care
• Perform thorough health assessments and care plan reviews
• Promote client education on health maintenance and prevention
Requirements:
• MD degree and active CPSO license
• At least two years of family medicine experience
• Commitment to serving diverse and vulnerable populations
• Familiarity with PS Suite EMR and MS Office
• Excellent communication and interpersonal abilities
Utilize your skills to support vulnerable communities through improved healthcare access and education at PQCHC.
