Placement Coordinator

Position: Placement Coordinator - 3 Month Contract

Job Description

Our client in the Non-Profit/Health Care Sector is currently seeking a Placement Coordinator to join their team on a contract basis starting on a 3 month contract. This role will be onsite in Westboro, with hours Monday-Friday 7am-3:30pm.

The successful candidate will come with previous experience working in the medical field.

Responsibilities:

• Collaborate with Champlain LHIN (Local Health integration Network) to maximize full occupancy and attend meetings, as needed

• Review all resident applications for admission to determine appropriate level of care; assist with processing of admission and discharge papers

• Maintain the waitlist and internal transfer list

• Coordinate internal transfers including the update of documentation

• Process paperwork for deceased residents

• Provide Resident Admission Packages for Families and Nursing

• Ensure all Resident Health Cards are updated: renewed, cancelled or address changed

• Meet with families and help coordinate smooth transition for admissions including the review and revision of the Admission Agreement

• Complete and submit High Intensity and Lab Claims forms in a timely manner and ensures funds received

• Handles and ensures the protection of extremely confidential and sensitive information and material

Requirements:

2 years of experience working in Healthcare

1 year of Administrative/Coordinator experience

Empathetic and strong communication skills

Organized and effective time management
