• Provide outreach to members via phone or home visits to discuss care/service plans and address questions or concerns.
• Coordinate care activities with healthcare and community providers to accommodate changes in member needs.
• Support various inquiries and requests related to member care plans.
• Document and maintain member records in compliance with state and regulatory requirements.
• Identify needs and make referrals to Care Managers and community organizations.

• High School diploma or GED.
• 1-2 years of related experience in healthcare or social services.
• Valid driver's license and ability to pass background checks for certain locations.
• Strong communication skills for effective interaction with members and providers.
• Ability to work collaboratively in a team-oriented environment.

• Experience working with youth and families in foster care or social services.
• Familiarity with trauma-informed care practices.
• Skills in using applications like Outlook, Excel, and other relevant software.
• Knowledge of community resources available to support members.
• Previous experience in care coordination or case management.
