Team Choice is looking for RPN Care Coordinator to fill an existing position.
Team Choice is seeking a bilingual, dedicated and experienced RPN Care Coordinator to join our team. Reporting to the Director of Care, the RPN Care Coordinator is responsible for planning, directing, and evaluating patient and staff, implementing standards of care, and development of field staff personnel. The Care Coordinator provides leadership and direction in all aspects of patient care. This individual will assist in the delivery of services including patient intake, assessments, delegations, scheduling, education, complaints, and ongoing monitoring.
KEY RESPONSIBILITIESPatient Care & Coordination
- Complete patient onboarding process in English or French as required.
- Develop and maintain effective Care Plans to meet established goals.
- Assure that the level of care established for patients is safe and appropriate and communicate with Ontario Health a Home with any issues or concerns.
- Consult with patients, families, and Ontario Health at Home in matters relating to patient care as needed.
- Maintain accurate documentation including approvals of shift reports, progress notes, risk codes, schedules, and goals.
- Ensure the level of care provided is safe, appropriate, and compliant with regulatory standards.
- Respond to staff, client and family requests/concerns/incidents/complaints and complete incident investigations, action plans, disciplinary actions and/or follow-ups as required.
- Promote and maintain positive public relations and quality care by participating in joint meetings, care conferences, visits with patients.
- Conduct patient satisfaction calls and maintain appropriate records and logs.
- Investigate complaints, issues and concerns, reports to management and follow up promptly following established procedures.
- Perform additional duties as required.
- Support and advocate for field staff by responding to concerns and requests.
- Approve, train, and monitor special functions as outlined in delegation procedures and job descriptions.
- Perform supervisory visits as needed for but not limited to training, delegation, safety, and performance issues.
- Ensure appropriate interventions and/or follow-up actions are implemented and completed.
- Oversee scheduling coordinators to ensure that patients are properly scheduled in a customer-friendly and efficient way.
- Supervise and evaluate Personal Support Worker performance and conduct disciplinary actions when required.
- Support Personal Support Workers to meet Quality Assurance, Infection Control and Risk Management policies, procedures, education, and practices.
- Provide ongoing orientation, coaching, mentoring, and support with patient care web-based applications.
- Registered Practical Nurse with current unencumbered licensure to practice professional nursing in the Province of Ontario.
- Thorough knowledge of Personal Support Worker processes and practices.
- Previous experience with Ontario Health at Home programs and processes is desirable.
- Excellent interpersonal skills and critical thinking skills.
- French and English proficiency is an asset.
- Excellent customer service skills and ability to deal tactfully with patients, staff and community in English and French.
- Ability and willingness to travel in Ottawa and surrounding area.
- Ability and willingness to be on call 1 week (7 days) every five weeks to provide phone support.
- Must be able to complete client home visits and have access to a reliable vehicle.
