The physiotherapist will work within the West End Integrated Falls Prevention Program addressing individual needs through assessment and evidence-based interventions. This role involves educating caregivers and offering tailored health strategies, ensuring clients receive comprehensive care in community settings.
Key Responsibilities:
• Implement client-centered evaluations for falls prevention
• Develop strategic treatment plans collaboratively
• Educate clients and caregivers on health practices
• Document client progress accurately and promptly
• Engage in community outreach efforts for falls prevention
Requirements:
• Master's Degree in Physiotherapy
• Registration with the College of Physiotherapists of Ontario
• Three years of physiotherapy experience in community settings
• Experience with fall prevention strategies
• Valid driver's license and insurance
Deliver impactful physiotherapy care and contribute to community health improvement at PQCHC.
