LMC Healthcare seeks a Diabetes Educator to empower patients in Ottawa through comprehensive education and support. This full-time position is ideal for a Registered Dietitian or Registered Nurse eager to assist in diabetes management initiatives.
As part of Canada's leading diabetes care provider, you’ll engage with patients in a multidisciplinary environment to promote healthier lifestyles. You will educate on self-management skills, lead workshops, and collaborate on care plans tailored to individual needs. Your contributions will enhance the quality of diabetes care across the community,
Key Responsibilities:
• Teach self-management skills for diabetes management
• Facilitate care plan development
• Input patient information into EMR efficiently
• Provide telehealth follow-ups and support
• Conduct community outreach and wellness events
Requirements:
• Credentialing with the College of Dietitians of Ontario
• Experience in educational or counseling settings
• Working towards CDE certification
• Access to a personal vehicle
• Skills in motivational interviewing
