Key Responsibilities:

• Document patient cases per Ontario Health atHome policies
Requirements:
• Registered Nurse, Physiotherapist, Occupational Therapist, or related degree
• Minimum two years of relevant community-based experience
• Fluency in English and French is required
• Valid driver’s license with vehicle access
• Completed Vulnerable Sector Check within six months
