As a Care Coordinator, you will manage a varied patient caseload and assess their health needs for personalized care plans.
Your role involves collaborating with patients and caregivers to enhance their understanding of treatment options while coordinating home and community care services. You will ensure beneficial connections across healthcare providers for optimal patient outcomes.
Key Responsibilities:
• Conduct comprehensive patient assessments and evaluations
• Develop individualized care and service plans
• Coordinate community health service delivery
• Facilitate communication within multidisciplinary healthcare teams
• Maintain accurate case documentation per policies
Requirements:
• RN or health-related degree with Ontario registration
• Minimum two years of community-related experience
• COVID-19 vaccination proof required
• Valid driver’s license and vehicle access
• Fluency in English;
