Ontario Health Care Coordinator Needed

This role provides the opportunity to work closely with patients and their families, assessing their health and psychosocial needs. As a Care Coordinator, you will create and implement individualized care plans while coordinating necessary resources. You'll connect patients with healthcare providers to enhance their wellbeing at home and beyond, all while being supported by a team of professionals.

Key Responsibilities:

• Assess and document patients’ clinical and psychosocial requirements

• Collaborate with care teams to create effective health plans

• Oversee service coordination from healthcare providers

• Facilitate patient understanding of care plans and goals

• Maintain compliance with documentation standards

Requirements:

• Degree in nursing, occupational therapy, social work, or equivalent

• Minimum two years community-based experience

• Proof of COVID-19 vaccination is required

• Strong verbal and written communication skills

• Valid driver's license and access to a vehicle
