As a vital part of the Acute Care Transitions Program, you will manage care flow between hospitals and community teams while enhancing patient outcomes. Key Responsibilities:

• Serve as the main contact for hospital coordinators

• Manage client tracking and flow tools

• Accept and review referrals for transition services

• Coordinate with SE @Home Team and hospitals

• Participate in complex case discharge meetings

Requirements:

• Fluent in French and English, both written and oral

• Membership with a regulatory body in Ontario

• 3+ years experience in community health

• Strong interdisciplinary case management skills

• Proficient in Microsoft Office and EHR systems
