As a Care Coordinator, you will assess diverse patient needs, collaborating closely with caregivers and healthcare teams to enhance patient engagement and service accessibility. Your focus will be on developing effective care plans and coordinating services that address both clinical and psychosocial challenges for patients. Key Responsibilities:

• Perform detailed assessments of patient and caregiver needs

• Create customized care plans with patients and families

• Oversee delivery of home health services

• Ensure effective communication among healthcare providers

• Document cases in accordance with legislative policies

Requirements:

• Registered health professional with Ontario certification

• Two years of relevant community experience preferred

• Mandatory COVID-19 vaccination proof

• Access to a vehicle with a valid driver’s license

• Bilingual capabilities in English and French are preferred
