As a key player in Carefor's community support team, you will work closely with clients to assess needs and create personalized rehabilitation plans. Your efforts will focus on restoring physical function while providing education on self-care. This role thrives on making connections and celebrating recovery milestones in your clients' lives.

Key Responsibilities:

• Assess and treat clients with diverse needs

• Implement evidence-based rehabilitation strategies

• Develop and track personalized care plans

• Train clients in using mobility aids and equipment

• Collaborate with interdisciplinary team members

Requirements:

• Good standing registration with the College of Physiotherapists of Ontario

• Strong rehabilitation focus and clinical expertise

• Excellent communicative abilities to engage clients

• 2-5 years of relevant clinical experience preferred

• Reliable vehicle and valid driver’s license
