Key Responsibilities:
• Complete thorough assessments of patients' health needs

Requirements:
• Registered professional in Nursing, Occupational Therapy, or related field
• Possess two years of applicable community experience
• Required fluency in both English and French
• Must hold a valid driver’s license and vehicle access
• Completed a Vulnerable Sector Check in the last six months
