As a Bilingual Float Care Coordinator, you'll engage directly with patients, their caregivers, and healthcare partners. Your responsibilities include determining eligibility for services, devising individual care plans, and ensuring effective transitions from hospital to home. You will play a key role in supporting health services while adhering to provincial guidelines, creating a significant impact in the community.
Key Responsibilities:
• Conduct thorough patient assessments and identify resource needs
• Collaborate on patient-centered care and service planning
• Coordinate community resources and home care services
• Facilitate multidisciplinary team cooperation for better outcomes
• Manage discharge planning and ongoing patient reassessment
Requirements:
• Credential as a Registered Nurse, Therapist, or Social Worker
• Two years of relevant work experience is essential
• Must be fluent in English and French for effective communication
• Required N95 Mask Fit Test documentation
• Complete Vulnerable Sector Check within six months
