In this key position, the Bilingual Transitions Care Lead will bridge hospital and community teams to support safe patient transitions. You will play a critical role in client care, demonstrating strong leadership and coordination within diverse healthcare settings. Key Responsibilities:

• Oversee the client tracking and notification processes

• Coordinate with hospital staff for quality care

• Draft initial care plans based on patient needs

• Participate actively in hospital discharge conferences

• Monitor patient outcomes and report accordingly

Requirements:

• Must be bilingual in French and English

• Minimum 3 years of experience in community health

• Advanced skills in Microsoft Office and EHR systems

• Valid driver’s license and access to transportation
