As a Care Coordinator, you will manage a diverse set of patients and engage in comprehensive assessments to devise individualized care plans. Your efforts will ensure connected, patient-centred services while working together with a supportive healthcare team.
Key Responsibilities:
• Execute thorough patient assessments using standardized tools
• Create and implement individualized care plans
• Coordinate service delivery for home and community care
• Communicate effectively within the healthcare team
• Maintain accurate case documentation according to policies
Requirements:
• Qualified in nursing, physiotherapy, or a related field
• Minimum of two years’ experience in community care
• Proficiency in English;
French speaking is an asset
• Proof of COVID-19 vaccination required
• Must possess a valid driver's license and vehicle access
