This role is designed for an experienced Occupational Therapist skilled in assessments and interventions across physical, cognitive, and psychosocial domains. You will support a diverse client population, conducting assessments and implementing intervention plans while working within an interdisciplinary healthcare team. Your focus will be on improving function and safety through community outreach and home visits.

Key Responsibilities:

• Conduct thorough, client-centred assessments

• Create and execute individualized intervention strategies

• Assess home safety and fall risks for clients

• Provide health promotion and disease prevention strategies

• Support accessibility to disability benefits for clients

Requirements:

• Master's or Bachelor's Degree in Occupational Therapy

• Current registration with the College of Occupational Therapists of Ontario

• Minimum three years of relevant experience

• Strong skills in clinical documentation and care planning

• Experience with culturally diverse communities
