CBI Home Health is seeking a dedicated Physiotherapist to support patient recovery and mobility post-hospitalization. This role focuses on delivering quality home care within a flexible schedule, enhancing patient outcomes while allowing you to manage your professional and personal life effectively. You will work collaboratively with interdisciplinary teams and have the resources you need for success.
Key Responsibilities:
• Deliver community-based physiotherapy for patient recovery
• Implement plans following hospitalization to improve outcomes
• Collaborate with interdisciplinary teams for holistic care
• Travel to client homes providing personalized therapy
• Document patient progress and adjust care as needed
Requirements:
• Registered with the College of Physiotherapists of Ontario
• Valid driver’s license and vehicle for travel
• Strong communication and collaboration skills
• Commitment to person-centered care
• Empathetic and critical thinking in client services
