- Complete comprehensive client assessments, develop and implement care plans based on evidence‑based practices and client needs.
- Act as primary provider and point‑of‑care lead for assigned client caseloads, collaborating with team members and support functions.
- Support clients through care transitions by connecting them with community support services.
- Current registration with the College of Nurses of Ontario.
- Canadian Nurses Association (CNA) specialty certification valuable.
- Enthusiasm for community care and evidence‑informed practice.
- Current CPR certificate.
- Valid driver’s licence and vehicle (travel required).
- Updated immunization records; clear criminal background and vulnerable sector checks required upon hire.
- Willingness to attain required Saint Elizabeth certifications.
- Availability to work days, evenings, nights, weekends.
- Physical capability to lift up to 55 pounds and frequent movement.
- Not previously employed as a nurse in Ontario within six months prior to hiring.
- Hold a Certificate of Registration in good standing from the College of Nurses of Ontario.
- Begin ‑year commitment between April 1 2025 and March 31 2026.
- Commit to full‑time work schedule offered by the employer for two years.
- Not previously participated in the CCPN.
- Not simultaneously receive Ministry of Health Tuition Support Program funds.
- Not hired through the Nursing Graduate Guarantee program.
