. This position is part of the Community Commitment Program for Nurses (CCPN) and offers a $25,000 incentive for a 24‑month employment commitment.

• Complete comprehensive client assessments, develop and implement care plans based on evidence‑based practices and client needs.
• Act as primary provider and point‑of‑care lead for assigned client caseloads, collaborating with team members and support functions.
• Support clients through care transitions by connecting them with community support services.

• Current registration with the College of Nurses of Ontario.
• Canadian Nurses Association (CNA) specialty certification valuable.
• Enthusiasm for community care and evidence‑informed practice.
• Current CPR certificate.
• Valid driver’s licence and vehicle (travel required).
• Updated immunization records; clear criminal background and vulnerable sector checks required upon hire.
• Willingness to attain required Saint Elizabeth certifications.
• Availability to work days, evenings, nights, weekends.
• Physical capability to lift up to 55 pounds and frequent movement.

• Not previously employed as a nurse in Ontario within six months prior to hiring.
• Hold a Certificate of Registration in good standing from the College of Nurses of Ontario.
• Begin ‑year commitment between April 1 2025 and March 31 2026.
• Commit to full‑time work schedule offered by the employer for two years.
• Not previously participated in the CCPN.
• Not simultaneously receive Ministry of Health Tuition Support Program funds.
• Not hired through the Nursing Graduate Guarantee program.
