Location: pembroke
Embark on a fulfilling career as a Registered Nurse with Para Med, serving communities in Renfrew, Arnprior, Pembroke, and Deep River. This role allows for autonomy and personal connections with patients.
As a full-time nurse, you will conduct home visits to assess and implement personalized care plans for patients across different age groups. Your contributions will be pivotal in ensuring patients receive high-quality care while maintaining their comfort and independence laborate with healthcare teams and build lasting relationships with your patients and their families.
Key Responsibilities:
• Provide comprehensive nursing assessments in home settings
• Develop and monitor care plans tailored to individual needs
• Collaboratively implement health interventions with teams
• Conduct health education for patients and families
• Document patient progress and changes effectively
Requirements:
• Nursing degree and CNO registration required
• Clear Vulnerable Sector Screening certificate
• Valid driver's license with access to a vehicle
• Experience in community health is beneficial
• Willingness to travel for patient care needs
