Position: Michigan Nurse Practitioner Role

As Porter expands into Michigan, we are seeking skilled Nurse Practitioners to conduct vital health assessments within their communities.

Your role will involve evaluating medical histories, performing vital sign tests, and actively participating in patient care management. If you are passionate about improving healthcare access, your contributions will be invaluable in guiding patients toward independence.

Key Responsibilities:

• Execute detailed in-home assessments and evaluations

• Conduct vital signs and medical history reviews

• Collaborate on care management with Porter Care Guides

• Perform point-of-care testing as necessary

• Communicate effectively with patients and staff
Requirements:

• Must hold a valid Nurse Practitioner license in Michigan

• At least three years of clinical experience, ideally in primary or geriatric care

• Excellent written and verbal communication skills

• Strong sense of patience and empathy

• Willingness to travel within geographic area
