What will you do?

The Nurse Practitioner is a self-directed practitioner who provides holistic patient care based on advanced nursing and basic medical management knowledge and skills for a selected patient population. You will:

  • Provide care for medically complex patients and palliative care for patients in their own homes as they near end of life.
  • Liaise with patients, families and all health care providers, using clinical assessment, monitoring and management skills to provide the best possible patient care.
  • Help patients manage pain and symptoms and avoid unnecessary hospitalization, as well as manage acute and episodic episodes of complex disease.
  • Delivers comprehensive primary health care services (including medication reconciliations, prescribing medication, ordering lab work, chart reviews, physical exams) to at-risk adults and seniors facing access barriers to health care.
  • Liaises with existing patient physicians and other community partners to address patient needs/goals.
  • Provides health promotion and education to patients in the Specialty Programs using diverse methods tailored to each patient’s needs.
  • Educates patients on disease prevention and prevention of further disability.
  • Contributes to care planning and supports the Care Coordination team with recommendations based on assessments.
  • Provides consultation and support to Ontario Health atHome Champlain Care Coordinators and key stakeholder partners.
  • Collaborates with Specialty Program leaders to ensure best practice and continuous improvement.
  • Supports development and implementation of Ontario Health atHome Champlain policies and procedures with community partners.
  • Acts as a Specialty Program advocate and coordinates services through ongoing liaison with committees as required.
  • Participates in regular professional development activities such as academic rounds and other educational events.

What must you have?
  • Membership, in good standing, with the College of Nurses of Ontario
  • Registered Nurse in the Extended Class (RN EC designation)
  • Working knowledge of community resources and roles of health care professionals
  • Ability to review discharge care plans and confirm outstanding medical tests and transportation arrangements
  • Conducts medication reconciliation and reviews medication protocol with patients and caregivers
  • Initiates contact with primary care physician and provides updates on patient acute care events and post-discharge regime; facilitates a one-week follow-up with the PCP
  • Identifies patients requiring accelerated assessment and home care services and coordinates with the Care Coordinator
  • Informs and supports the Care Coordinator in developing the patient’s care plan and ensuring smooth transfers to ongoing care teams
  • Answers questions or concerns of patients and caregivers
  • Represents OH atHome as required and maintains positive public relations and coordination of services
  • Ensures a safe environment for patients, caregivers, family members, and staff; adheres to OH atHome health and safety policies
  • Participates in establishing and monitoring standards for Case Management and quality initiatives
