Experienced Nurse Practitioner in Palliative Care

In this position with the Regional Palliative Consultation Team, you will engage with complex patients, applying your advanced skills to deliver comprehensive health assessments, pain management, and symptom control. Working closely with families and community partners, you will ensure that patients receive the best possible care in their home settings.
Key Responsibilities:
• Deliver primary health care services to at-risk patients
• Collaborate with physicians and care coordinators
• Facilitate patient education on health management
• Conduct focused assessments for care initiatives
• Promote effective coordination of health services
Requirements:
• Good standing with the College of Nurses of Ontario
• RN EC designation required
• Knowledge of health care protocols and community roles
• Palliative/End of Life care experience preferred
• Strong communication skills
Provide compassionate and effective palliative care with Ontario Health atHome, making a difference in patients' lives.
