Outreach Nurse Practitioner

Your primary responsibility will be delivering holistic, primary health care to marginalized populations. This role demands strong clinical skills for assessment, treatment, and education while ensuring safety through harm reduction practices.

Key Responsibilities:

• Conduct patient assessments and develop care plans

• Maintain accurate patient documentation and history

• Collaborate with nurse coordinators and community services

• Engage in outreach to provide care in diverse settings

• Provide necessary health education to staff and clients
Requirements:

• Current Registered Nurse (EC) credential from CNO

• 2-5 years’ experience caring for vulnerable populations

• Excellent interpersonal and communication skills

• Familiarity with medical software and EMR

• Must have up-to-date vaccinations and CPR certification
