RN Case Manager

Position: RN Case Manager Wanted
Join Bayshore Medical Personnel as a Full-Time RN Case Manager in Ottawa, Ontario and make a difference in patient care. This role is focused on managing complex health cases and provides a stable daytime schedule.

As an RN Case Manager, you will work within a primary health care team to perform case assessments and develop customized care plans that respond to the unique needs of clients.

Your role will encompass ongoing monitoring and adjustments of care as situations evolve. Benefit from a positive work/life balance with daytime shifts, free from evening and weekend obligations.

Key Responsibilities:

• Execute comprehensive assessments for clients with complex needs

• Design and apply individualized care plans based on assessments

• Monitor client health and suggest necessary modifications

• Collaborate with a team to ensure holistic care

• Adhere to all applicable nursing standards and regulations

Requirements:

• Baccalaureate Degree in Nursing required

• Certificate in Basic Life Support or CPR/AED Level C

• Active provincial nursing license necessary

• Minimum of 3 years clinical nursing experience required

• Previous case management background preferred
