As a Care Manager, you will oversee primary care management while developing tailored care plans for clients. The ideal candidate will have a strong nursing background and the ability to coach and mentor new graduates. Your contributions are essential for fostering a positive care environment.
Key Responsibilities:
• Provide primary care management and nursing support
• Create and oversee dynamic care plans for clients

Requirements:
• CNO registration in good standing required
• 1–2 years of nursing experience desired
• Bilingual in English and French a plus

• Valid driver’s license necessary
