Community Health Nurse Practitioner

Position: ConnectWell Community Health Nurse Practitioner
Location: Carleton Place

Join Connect Well Community Health as a Nurse Practitioner and support vital health care in the community. Work at both our Lanark and Carleton Place sites in a collaborative environment.

At Connect Well, we’re looking for an experienced Nurse Practitioner committed to providing exceptional primary care. You will work within an inter-disciplinary team, focusing on client education, health promotion, and self-management. Ideal candidates will have relevant qualifications, including a Master's degree in Nursing and effective leadership skills.

Key Responsibilities:

• Serve clients with comprehensive primary health care

• Work closely with the Primary Care Team

• Advocate for health education and self-management

• Perform on-call telephone duties as needed

• Follow the Standards of Practice from the College of Nurses of Ontario

Requirements:

• Completion of recognized NP program

• Registered as an RN (EC) in Ontario

• Two years’ experience preferred in primary care

• Demonstrated leadership in health practice

• Access to a vehicle and valid driver's license
