SE Health is looking for a passionate Registered Practical Nurse to join their interdisciplinary team.
Your role emphasizes client-centered care planning and the delivery of comprehensive support to ensure smooth transitions from hospital to home. Work collaboratively to assess client needs and engage with community support services for optimal care.
Key Responsibilities:
• Provide comprehensive care as part of a multidisciplinary team
• Serve as primary contact for designated clients and families
• Conduct thorough client assessments and develop care plans
• Lead discussions on care planning among team members
• Connect clients with essential community support services
Requirements:
• Registered with the College of Nurses of Ontario
• Minimum two years of nursing experience required
• Background in home/community or acute care
• Strong clinical leadership and assessment skills
• Valid driver’s license and willing to travel
