Nurse Practitioner; LTC
Overview
We are seeking a dedicated and compassionate Nurse Practitioner to join our team. The ideal candidate will have experience working with geriatrics, be able to work independently with minimal oversight, possess strong customer service skills, be seeking a long‑term and stable position, and have strong communication skills.

- Completion of recognized Nurse Practitioner program
- Current extended class certification with College of Nurses of Ontario
- Experience with chronic disease management and geriatric care

- Management of stable complex chronic co‑morbidities in collaboration with MDs
- Collaboration with an interdisciplinary team in the course of care delivery
- Provide high quality customer service
- Provide clinical / medical education to clients and staff
- Participate in care conferencing
- Role is about 70% clinical and 30% quality improvement, education, and leadership
