Position: Ontario Health Nurse Practitioner Role
Become a key player in patient care as a Nurse Practitioner with Ontario Health in Labelle. This role centers on delivering accessible, compassionate care to medically complex patients.

As a valued member of the Rapid Response Team, you will provide essential health care services, including palliative care for patients r responsibilities encompass pain management, medication reconciliation, and collaborating with multidisciplinary teams to achieve optimal health outcomes. You will also lead patient education initiatives tailored to individual needs, supporting health promotion and effective disease management.

Key Responsibilities:

• Provide comprehensive health care services to patients

• Coordinate with health care providers and community partners

• Educate patients on managing their health

• Conduct assessments and recommend care strategies

• Advocate for best practices and improvements

Requirements:

• Membership in good standing with Ontario’s nursing college

• RN in Extended Class designation

• Community resource knowledge

• Palliative care experience beneficial

• French fluency is required
