- Registered Nurses (RN) hourly rate $32.08–$47.38; progression based on hours worked and role‑specific criteria.
- Registered Practical Nurses (RPN) hourly rate $27.17–$33.37; progression based on hours worked and role‑specific criteria.
- Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Home and Community Care Support Services (HCCSS).
- Working with patients (both in‑person and virtually) to provide exceptional nursing services.
- Providing services in a variety of settings, which may include in‑home, clinic, retirement home or school.
- Communicating and liaising with patients, family members and others within the interdisciplinary care team.
- Registered Practical Nurse (RPN) diploma or Registered Nurse (RN) degree.
- Current College of Nurses of Ontario (CNO) registration in positive standing.
- Valid driver's license and vehicle (regular travel required).
- Excellent English verbal and written communication skills.
- Current immunization record, including a 2‑step TB test.
Care Partners In Your Community. In addition to providing home‑based health care, Care Partners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we've been organizing staff‑led medical care and clinics in countries with poor access to health care since 2009.
