Nurse Practitioner - Ambulatory Care Program

Brockville General Hospital is hiring a Temporary Full-Time Nurse Practitioner for the Ambulatory Care Program. Utilize your expertise in outpatient care and chronic disease management to benefit our patients.

This role emphasizes accountability in providing quality care within various ambulatory services while collaborating with an interdisciplinary team. Candidates must have a Master's degree in nursing, RN Extended Class registration, and relevant outpatient experience. Key Responsibilities:

• Provide direct care for patients in ambulatory settings

• Work with health care teams to ensure coordinated care

• Lead initiatives for quality improvement in ambulatory services

• Communicate effectively with primary health care providers

• Support patients in chronic disease management

Requirements:

• Master’s degree in nursing from an accredited institution

• Active registration with the College of Nurses of Ontario

• Minimum of 3 years nursing experience in relevant settings

• Strong independent worker with collaboration skills

• Proof of COVID-19 vaccination required
