Geriatric Nurse Practitioner

Position: Geriatric Nurse Practitioner Role
Advance your nursing career as a Nurse Practitioner focused on geriatric mental health care. This position emphasizes independent practice in comprehensive assessments and intervention strategies.

Reporting to the Program Manager, this Nurse Practitioner role requires a Master's degree in nursing or a BScN with specialty certification. With a specialty in Primary Health Care or Adult care, you will conduct intake assessments and develop care plans alongside an interdisciplinary team. This role also includes coordinating patient flow and managing EHR documentation.

Key Responsibilities:

• Conduct intake assessments and medication reconciliations

• Develop and implement care plans collaboratively

• Coordinate patient flow and triage referrals

• Monitor and prescribe psychiatric medications

• Lead health promotion initiatives for patients

Requirements:

• Master's degree or BScN with NP certification

• 5 years of relevant nursing experience

• Registration as a Registered Nurse in Ontario

• Competency in health assessment and management

• Strong communication skills in English and French
