In this role, you will be a vital member of the healthcare team, supporting diverse communities in achieving health goals. With your background in nursing and experience in public health settings, you will provide comprehensive care, facilitating programs aimed at health education and disease prevention. You will work collaboratively to address the needs of marginalized populations and contribute to quality improvement initiatives.
Key Responsibilities:
• Deliver clinical care, including assessments and education
• Conduct telephone triage and coordinate referrals
• Engage in community outreach and client health promotion
• Document client interactions and ensure confidentiality
• Support the development and implementation of health programs
Requirements:
• Degree in Nursing from an accredited institution
• 3-5 years of experience in community health
• Active registration with the College of Nurses of Ontario
• French language proficiency required, with additional languages an asset
• Experience working with diverse populations
