In this position, the Nurse Practitioner reports to the Program Manager and is responsible for providing advanced nursing care to older adults. You will carry out detailed intake assessments, determine program eligibility, and create individualized care plans with the team. Your contributions will also involve managing referrals and maintaining precise electronic health records.

Key Responsibilities:

• Perform comprehensive mental health and medical assessments

• Determine program eligibility and manage admissions

• Lead and coordinate the discharge planning process

• Provide primary psychiatric care and medication management

• Advocate for patients through health education initiatives

Requirements:

• Master’s degree in nursing with NP certification

• Minimum of 5 years nursing experience

• Current nursing registration in Ontario

• Knowledge of relevant health legislation

• Proficient communication skills in both English and French
