In this crucial role, you will assess clients and develop tailored care plans, ensuring best practices are followed. You'll serve as the primary care lead for assigned clients while actively collaborating with various care team members. Supporting children through transitions, you’ll connect families to vital community services.
Key Responsibilities:
• Conduct comprehensive client assessments and develop individualized care plans
• Serve as primary provider, accountable for client care
• Collaborate with team members to ensure seamless care transitions
Requirements:
• Registration with the College of Nurses of Ontario
• CNA specialty certification preferred
• Strong interpersonal and communication skills
• Current CPR certificate is required
• Valid driver’s license; travel required
