Senior Nurse Practitioner in Geriatrics

Reporting directly to the Program Manager, the Nurse Practitioner will provide advanced clinical nursing services within a specialized mental health framework. This includes conducting thorough assessments, crafting care plans, and overseeing the patient flow effectively. Collaboration with a professional interdisciplinary team is key to ensuring continuity of care for patients.

Key Responsibilities:

• Conduct initial psychiatric and medical intake assessments

• Collaborate with the interdisciplinary team on care plans

• Manage patient documentation within EHR systems

• Provide oversight in psychiatric medication management

• Take the lead in patient education and health promotion

Requirements:

• Master’s degree in nursing or equivalent with NP certification

• At least 5 years of clinical experience

• Registration as an Extended Class Nurse in Ontario

• In-depth understanding of pertinent healthcare legislation

• Excellent communication in English and French
