As a member of SE Health, you will provide essential home and community care for children and families, collaborating with CHEO to ensure top-tier care. This role requires you to conduct assessments, develop tailored care plans, and support clients through transitions to community services. You’ll work as part of a dedicated care team, making a significant difference in children’s lives.
Key Responsibilities:
• Perform comprehensive client assessments and develop individualized care plans
• Serve as the primary care provider accountable for assigned clients
• Facilitate smooth care transitions by connecting with community support services
Requirements:
• Current Certificate of Competence with the College of Nurses of Ontario
• Up-to-date immunization and background checks
• Willingness to complete SE Health certifications
• Valid CPR certification
• Ability to meet physical demands of the job
