SE Health partners with CHEO to provide home and school care to children and families. The School Nurse plays a critical role in the child’s care team by delivering evidence‑based nursing care across home and school settings.
PositionFull Time or Part Time Registered Practical Nurse (RPN) – School Nurse.

- Complete comprehensive client assessments, develop individual care plans, and implement and evaluate care based on best evidence and client needs.
- Act as the primary care lead for the client caseload, collaborating with the care team and support functions.
- Coordinate care transitions by connecting clients with community support services.
- Current registration with the College of Nurses Ontario.
- Canadian Nurses Association specialty certification is an asset.
- Strong commitment to community care, evidence‑informed practice, and excellence in care.
- Excellent organizational, time‑management, and communication skills (oral and written).
- Ability to adapt to rapidly changing demands and work independently as well as part of a team.
- Demonstrated appreciation for Quality Improvement processes.
- Current CPR certificate required.
- Valid driver’s licence and vehicle required; travel necessary.
- Updated immunization records, clear criminal background check, and vulnerable sector check required upon hire.
- Willingness to attain required Saint Elizabeth certifications.
- Availability to work weekdays, evenings, nights, and every other weekend.
- Physical ability to perform duties that involve lifting up to 55 pounds, walking, climbing stairs, standing, bending, and frequent movement.
