SE Health partners with CHEO to provide home and school care to children and families. The School Nurse plays a critical role in the child’s care team by delivering evidence‑based nursing care across home and school settings.

Position

Full Time or Part Time Registered Practical Nurse (RPN) – School Nurse.

Responsibilities
  • Complete comprehensive client assessments, develop individual care plans, and implement and evaluate care based on best evidence and client needs.
  • Act as the primary care lead for the client caseload, collaborating with the care team and support functions.
  • Coordinate care transitions by connecting clients with community support services.
Requirements
  • Current registration with the College of Nurses Ontario.
  • Canadian Nurses Association specialty certification is an asset.
  • Strong commitment to community care, evidence‑informed practice, and excellence in care.
  • Excellent organizational, time‑management, and communication skills (oral and written).
  • Ability to adapt to rapidly changing demands and work independently as well as part of a team.
  • Demonstrated appreciation for Quality Improvement processes.
  • Current CPR certificate required.
  • Valid driver’s licence and vehicle required; travel necessary.
  • Updated immunization records, clear criminal background check, and vulnerable sector check required upon hire.
  • Willingness to attain required Saint Elizabeth certifications.
  • Availability to work weekdays, evenings, nights, and every other weekend.
  • Physical ability to perform duties that involve lifting up to 55 pounds, walking, climbing stairs, standing, bending, and frequent movement.
