In this part-time role, you'll be tasked with conducting assessments and crafting care plans that reflect the unique needs of each child. As the lead provider, you will coordinate care efforts with your team and ensure smooth transitions to community support services. Your passion for evidence-based practices will be essential in promoting health and well-being.
Key Responsibilities:
• Complete assessments and develop evidence-informed care plans
• Act as primary caregiver for your caseload of one child
• Connect clients to local community support services
Requirements:
• Registration with the College of Nurses of Ontario
• Excellent organizational and interpersonal skills
• Strong computer skills in MS Office
• Updated immunization records required
• Willingness to obtain relevant certifications
Support and empower students as a dedicated School Nurse with SE Health.
