As a Registered Practical Nurse with SE Health, you will be part of a compassionate and innovative social enterprise. You’ll carry out comprehensive client assessments, develop tailored care plans, and collaborate closely with care teams to ensure the best outcomes. Key Responsibilities:
• Conduct comprehensive client assessments for care planning
• Implement and evaluate evidence-based care plans
• Act as the primary provider for a designated caseload
• Collaborate with community support services for clients
• Adapt care to meet each client’s unique needs
Requirements:
• Current registration with the College of Nurses of Ontario
• CNA specialty certification is advantageous
• Passion for community care and evidence-informed practices
• Valid CPR certificate
• Driver’s license and vehicle required for travel
