As a Nurse Practitioner (NP), you will report to the Manager of Supportive Housing and Substance Use Health. Utilize your nursing expertise to provide primary care services and work collaboratively with interprofessional teams. Successful candidates will perform assessments, diagnoses, health promotion, and client support while embracing a culturally sensitive approach.
Key Responsibilities:
• Conduct comprehensive triage and assessments for clients
• Deliver health promotion and preventative care services
• Provide diagnosis and treatment within the scope of practice
• Collaborate with diverse health teams for effective care
• Maintain a progressive roster of responsible clients
Requirements:
• MScN RN(EC) or equivalent education
• Minimum two years’ experience as a Nurse Practitioner
• Unrestricted registration with the College of Nurses of Ontario
• Strong clinical skills in emergency and outreach environments
• Knowledge of harm reduction practices
