As a full-time RN, you will visit patients in their homes and cater to their unique health care needs. This role requires strong assessment skills and the ability to collaborate with interdisciplinary teams to ensure effective care. You'll also engage in developing long-term relationships with patients, contributing to their comfort and well-being.
Key Responsibilities:
• Assess patients’ care needs with appropriate tools
• Monitor care plans for effectiveness and compliance
• Build rapport with patients and their families
• Administer medications and document changes in condition
• Collaborate with health teams for coordinated care
Requirements:
• Nursing degree and CNO registration in good standing
• Valid CPR certification and driver's license
• Experience in community care is preferred
• Ability to travel to patient locations
• On-call availability for patient safety
