In this term part-time role, you will work 21 hours per week as part of the Integrated Health Services team. Your responsibilities will encompass working with a diverse patient population and ensuring accessible health services. Engaging with an interdisciplinary team, you will support patients across various clinics and help manage complex health issues.
Key Responsibilities:
• Provide primary health care services to clients across the lifespan
• Emphasize disease prevention and health promotion
• Participate in same-day access clinics and cover multiple clinics
• Collaborate with a multidisciplinary team of health professionals
• Apply a harm reduction approach in patient care
Requirements:
• Master’s degree in Nursing required
• Minimum two years' experience as Nurse Practitioner in community care
• Registered in the Extended Class with the College of Nurses of Ontario
• Strong clinical assessment and problem-solving skills
• Ability to communicate effectively with diverse populations
