Carefor Rapid Response Nurse Position

Join Carefor as a Rapid Response Nurse in Eastern Ontario, where you will make a meaningful impact by providing quality care to clients re-entering home life from hospitals.

In this role, you will report to the Manager of Transition Programs, overseeing care for clients within the atHome framework. Responsibilities include conducting assessments, engaging with multidisciplinary teams, and training personal support workers. Your efforts will contribute directly to minimizing unnecessary hospital visits by ensuring clients have appropriate home support, ultimately promoting their well-being and independence.

Key Responsibilities:

• Support nursing care for hospital transition clients

• Complete comprehensive assessments and care plans

• Mentor personal support workers on best practices

• Participate in team case conferences and evaluations

• Resolve client and staff issues effectively

Requirements:

• Active Registered Practical Nurse in Ontario

• Minimum three years of home health care experience

• Knowledge of Ontario’s legislative requirements

• Preferred certifications in First Aid and CPR

• Strong proficiency in Microsoft Office tools
