You'll collaborate with an interdisciplinary team, provide same-day access clinics, and engage with priority communities.

Key Responsibilities:

• Deliver primary health care services and disease management

• Participate in same day access clinics for diverse clients

• Collaborate with a multi-disciplinary health care team

• Support complex mental health and addiction care

• Apply a harm reduction approach to treatment

Requirements:

• Master’s degree in Nursing required

• Valid registration in the Extended Class, College of Nurses of Ontario

• Minimum two years Nurse Practitioner experience

• Strong clinical assessment and problem-solving skills

• Bilingual in French preferred
