Sandy Hill Community Health Centre seeks a Community Health Nurse to provide essential care in an integrated health program. This permanent, full-time role requires on-site commitment to client health services.
In this nursing role, you'll work closely with a dynamic team to facilitate health improvements for clients, utilizing your skills and knowledge to make a difference. Meeting the licensing requirements of the College of Nurses of Ontario is essential, along with the ability to adapt to diverse client needs.
Key Responsibilities:
• Deliver high-quality nursing care in clinic settings
• Manage and support clients with varying health needs
• Work collaboratively with health service teams
• Conduct both physical and mental health assessments
• Promote client education and health literacy
Requirements:
• Bachelor's degree in nursing or related field
• 3-5 years of community nursing experience
• Fluency in French and English
• Experience in harm reduction and mental health
• Strong assessment and counseling abilities
