Part of a nation wide family of Continuing Care Retirement Communities, Covenant Care at Home is seeking a Visit LPN/LVN. This position is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
• Uses equipment and supplies effectively and efficiently.
• Completes and submits all documentation according to agency policy and procedures.
• Participates in personal and professional growth and development.

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Community/home health or medical/surgical experience is preferred.
• Current licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Ability to comply with accepted professional standards and practice.
• Demonstrates good verbal and written communication, and organization skills.
• Demonstrates good basic computer skills.

$33.96 - $47.27 per hour

A reasonable estimate of the pay range for this position is $33.96 - $47.27 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
